Volunteer Statement of Confidentiality

Volunteers with the Bichon Frise Club of America Charitable Trust (BFCACT) will ensure confidentiality and privacy regarding information on the BFCACT related databases which includes, but is not limited to, applications for adoption forms and placements. All information received while serving as a volunteer for BFCACT is considered confidential. This information is strictly for the use of BFCACT and may not be used for any other purpose without written consent from the Board of Directors.

I have read and understood the above Statement of Confidentiality and will maintain confidentiality and privacy of information regarding other volunteers, applicants, forms, and other information of the BFCACT databases.
